Documents needed for adoption packet

(a) TWO original letters from both Prospective Parents employers stating their income per year and length of employment, which must printed on companys letterhead and sealed with a company seal, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the employers location;
(b) TWO copies of both Prospective Parents past three years tax returns (form 1040) including tax return for the year 2003, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(c) TWO original copies of the certificate of assets of the adoptive family signed by the certified U.S. accountant, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(d) TWO original copies of the marriage certificate signed by the authorized officer of the Vital Statistics Department in the state where the marriage was registered, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(e) ONE original copy of birth certificate for all family members signed by the authorized officer of the Vital Statistics Department in the state where the person was born, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(f) ONE copy of divorce decree for any of the Adoptive Parents [if applicable], notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(g) TWO original copies of the home study, including post placement commitment by the social worker or adoption agency and at least three letters of reference (non-family members), signed by the social worker or adoption agency which are licensed in the state of the Prospective Parents residence, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(h) TWO original copies of the social workers or adoption agencys license, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(i) TWO original proofs of ownership of Prospective Parents house (title or deed), including a copy of mortgage payment, balance of payments, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(j) TWO letters from authorized department in any particular state stating that checks on Prospective Parents (and their children above 18 years old) in FBI, SBI and Child Abuse Registry show no record, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(k) TWO original complete physician reports - each for both Prospective Parent, indicating that Prospective Parents do not have the following diseases: AIDS, psychological diseases, alcoholism, drug addiction, toxic addiction, tuberculosis, leprosy, cancer, sexually transmitted and infectious diseases, and are not physically handicapped mentally or mentally retarded, signed by the physician, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(l) ONE set of photographs of Prospective Parents family, relatives, and home;
(m) THREE color photocopies each of Prospective Parents passport. If Prospective Parents are applying for a new passport, they have to make sure they use their full names (first, middle, last name) and then make they use it consistently throughout all of their documents;
(n) ONE original copy of the INS form stamped by the Immigration and Naturalization Service, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(o) FOUR original powers of attorney authorizing lawyers to represent the Prospective Parents in all governmental and non-governmental organizations of the Republic of Kazakhstan, signed by both Prospective Parents, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(p) TWO original applications to the Kazakhstan Consulate in NY, NY signed by both Prospective Parents, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location;
(q) FOUR original affidavits wherein Adoptive Parents commit themselves to send to the Kazakhstan Consulate in NY reports on the childs progress every six months, signed by both Prospective Parents, notarized by the U.S. notary and certified by the apostille stamp issued by the Secretary of State in the state of the notarys location.
