COMMUNITY HOME-BASED INITIATIVES PROGRAM (CHIP) Sponsor: Division for Aging Services Phone: (702) 486-3545 Hours: Monday-Friday, 7:30 a.m. - 4:30 p.m. Provides alternative services to meet the needs of the senior who is at risk of institutionalization. Services are available for income eligible individuals age 66 and older. This program provides light housekeeping, shopping, errands, laundry, and standby assistance with bathing to individuals 60 and over or disabled. Individuals must be Medicaid or income eligible. The program staff also provides instruction in efficient methods of household management and methods of self-care to help compensate for physical limitations. Respite service for persons caring for dependent elderly family members is also provided. SOUTHERN NEVADA PERSONAL CARE ASSISTANCE SERVICE PROGRAM Sponsor: Accessible Space, Inc. (NCEP) 2820 W. The program is designed to assist people with such cares as bathing, dressing, personal hygiene, assistance with feeding, transfers, and essential laundry. No more than 28 hours per week of care can be provided to eligible persons. There is no sliding scale for payment. ALTERNATIVE HEALTH CARE PROGRAM Sponsor: Clark County Social Services Cambridge Community Center 3827 South Maryland Parkway Las Vegas, NV 89109 Phone: (702) 455-8646 The AHC Program provides carefully selected home health aides to assist recuperating patients to remain in their own homes following hospitalization for surgury, illness or injury. This short-term periodic assistance may typically include: personal care and grooming, grocery shopping, meal preparation, linen change, light housekeeping and/or personal laundry. The AHC Program is specifically designed to help preserve and restore individuals to independence as they regain strength and capability. Eligibility for this program is determined by low to moderate income guidelines. Clients must have been hospitalized within the last 30 days.
