To qualify a senior needs to meet these requirements....

Be 65 years of age or older
Be at risk for institutionalization
In need of one or more of the following:
bathing, dressing, eating, toileting,
ambulating, transferring, and
Be financially eligible ($1,692 income or less)
Once a referral is made, a social worker
will contact the person in need of services
to determine which services are most appropriate.
